1) Fill out Event Registration Form. Clearly indicate event (i.e., "2009 Sprint National Development Camp") in which athlete is participating. (One form per event per athlete.) Registration Form in .pdf format may be downloaded here>>

3) In order to meet the June 10, 2009 deadline, it is suggested that you send form and payment via FedEx, UPS or USPS Priority Mail.
4) Forms of payment accepted: check or credit card. (Please note on form).
5) If paying by credit card, you may also fax the Registration Form with credit card information to the USACK National Office at 704-348-4418, attn: Gerald.
6) Please ensure information on Registration Form is legible.
